There are four methods to renew your coverage:
- Renew online: Visit MyMedicaid.la.gov
- Renew by phone: Call the Louisiana Medicaid Hotline at 1-888-204-8032, Monday to Friday, from 8:00 a.m. to 4:30 p.m.
- Renew by mail: Send the completed form to Medicaid Application Office, P.O. Box 91278, Baton Rouge, LA 70821-9893.
- Renew in person: Contact your local Medicaid regional office for assistance.
